Medical Cannabis in 2026: Your Guide to Medical Treatment

Medical cannabis has been available on prescription in Germany since 2017, and the route to it can still feel confusing. So how does it work in 2026? This guide walks you through the process step by step, from finding a doctor to collecting your medication at a pharmacy.

*Important: A prescription is not guaranteed. It can only be issued after medical treatment and once all criteria are met. Whether that is the case is decided solely by the treating doctor. Fast pickup or delivery is only possible with a valid prescription.

Who Can Be Considered for Medical Cannabis?

Since 1 April 2024, Germany's medical cannabis framework has been the Medical Cannabis Act (MedCanG); before that, the 2017 "Cannabis as Medicine" law applied. Doctors may prescribe cannabis for "serious medical conditions". That's a fairly broad term, but the idea is clear: this isn't a treatment for the sniffles.

Conditions for which doctors commonly prescribe cannabis include:

  • Chronic pain, for example from osteoarthritis or fibromyalgia
  • Multiple sclerosis (MS), particularly for muscle spasticity
  • Loss of appetite and nausea during chemotherapy
  • Tourette syndrome
  • Certain forms of epilepsy
  • ADHD in adults

Important: Having a condition on this list does not guarantee a prescription. There is no automatic entitlement. A key requirement is that conventional treatments have either failed, caused intolerable side effects or been deemed medically unsuitable. The final decision always rests with your treating doctor, who will assess your individual case. You must be at least 18 years old and able to provide valid ID.

Your First Step: Finding the Right Doctor

Not every doctor in Germany is willing to prescribe cannabis. Many have reservations or simply lack experience with it. Although your GP can, in principle, issue a prescription, specialists such as pain management doctors, neurologists or palliative care physicians are often better placed to help.

So how do you find a suitable doctor? Online platforms specialising in cannabis treatment are a good place to start. Patient organisations and support groups can often recommend someone, too. These days, though, telemedicine is often the simplest route. Online platforms connect you directly with experienced doctors who are familiar with prescribing medical cannabis. More on that later.

Your Medical Consultation: How to Prepare

Your doctor's appointment is the most important part of the process. Good preparation makes all the difference. Don't just turn up and say, "I want weed on prescription." That won't get you far. You need to show your doctor that you've given serious thought to your condition and the available treatment options.

Here's what you should bring and how to approach the appointment:

  • All your medical records: Gather doctors' letters, diagnoses, test results and a list of all the medications you've tried. The more complete your documented medical history, the better.
  • A symptom diary: For a few weeks, keep a record of your symptoms, how severe they are and what you've done to manage them. This helps your doctor get a clearer, more objective picture of what you're experiencing.
  • Be honest and open: Describe your situation accurately. Explain why you think cannabis might help and why previous treatments haven't worked for you.

A well-prepared patient is more likely to be taken seriously. It shows that your priority is relief from your symptoms, not something else. Remember: your doctor needs to be able to justify the treatment to your health insurer and under the law.

Your Prescription: Private or Statutory Insurance

If your doctor decides after treatment that cannabis therapy is appropriate and all criteria are met, they can issue a prescription. There are two types you should know about.

Private Prescriptions

A private prescription is often issued at the start of treatment. This means you pay the cost of the cannabis at the pharmacy yourself. Costs depend on the strain, the quantity and the pharmacy.

Statutory Insurance Prescriptions

Since April 2024, medical cannabis is no longer a controlled narcotic, so a yellow BtM prescription is no longer required. If your statutory health insurer (GKV) covers your treatment, you receive your cannabis on a regular insurance prescription and pay the usual statutory co-payment. Getting that coverage approved, however, is a separate process.

Getting Your Health Insurer to Cover the Cost

For statutory health insurance to pay, your doctor must submit an application for coverage of cannabis treatment to your insurer. The application must explain in detail why the treatment is necessary for you. Your doctor must demonstrate that you have a serious medical condition and that no other standard treatment is available or effective.

Your insurer then reviews the application. Unfortunately, it's not unusual for applications to be rejected initially. If that happens, you can appeal. The process can be lengthy and stressful, which is why many patients start by paying for treatment themselves while their application is being processed.

In Germany, medical cannabis is available exclusively through pharmacies. Other countries, such as Spain, have different systems. There, you'll find private lounges and associations.

This system does not currently apply in Germany, but it shows just how much access rules vary across Europe. While patients in Germany depend on doctors and pharmacies, other countries have models that place more emphasis on community and personal interaction. You can learn more about the different locations and their rules on our website.

Getting Your Cannabis from a Pharmacy

Once a valid prescription exists, whether private or insurance-funded, your next stop is a pharmacy. In Germany, medical cannabis is only available from pharmacies. You can use any pharmacy, but it's worth choosing one with experience helping cannabis patients.

Not every pharmacy stocks every strain or product. In most cases, your medication will need to be ordered, which can take some time. You can call ahead to ask whether your prescribed product is in stock or how long it will take to arrive.

Your doctor can prescribe several forms of medical cannabis:

  • Cannabis flower: Dried, whole flowers for inhalation using a vaporiser. Smoking is not medically recommended.
  • Cannabis extracts: Oils or drops taken orally. These allow very precise dosing.
  • Ready-made medicines: Cannabinoid-based medicines such as Sativex, an oral spray, or Canemes capsules.

Your doctor specifies the exact strain and quantity on your prescription. You can't simply choose something else at the pharmacy.

Telemedicine with ZazaXpress: Treatment, Express Pickup or Delivery*

Does the process sound complicated? Telemedicine is an alternative to searching for a local practice. We work with zazaxpress.com, a platform that connects patients with doctors who have experience in cannabis therapy.

You can avoid the search for a local doctor and start the treatment comfortably from home.

Here's how it works:

  • 1. Sign up: Register on the website and first complete a medical questionnaire. Your treatment request is usually processed within 15 to 60 minutes*.
  • 2. Video call (voluntary): A video call with a doctor licensed to practise in Germany who specialises in cannabis treatment is voluntary.
  • 3. Medical decision: After reviewing your information and the medical treatment, the doctor decides whether a prescription is appropriate. A prescription is not guaranteed and is only issued once all criteria are met.
  • 4. Pickup or delivery*: With a valid prescription, the medicine is ready for pickup at the pharmacy* or can be delivered*. ZazaXpress offers express pickup or delivery* in Hamburg, Bremen, Berlin, Cologne, Osnabrück, Dortmund, Augsburg and Leipzig. Munich, Stuttgart and Düsseldorf are coming soon. In all other cities the medicine is shipped by DHL with a valid prescription, so it can be pre-ordered*. How long shipping takes depends on the individual case. Processing time, availability and delivery time depend on location and pharmacy.

Medical patients can request a prescription on zazaxpress.com and use the code ZAZA to save 50% on the platform fee. You must be at least 18 years old and have valid ID ready.

Separately from the medical route, Zazapass lists 420-friendly lounges in Hamburg, Berlin, Bremen, Stuttgart, Munich, Kaiserslautern, Wiesbaden, Worms and Neu-Isenburg. A lounge and the medical route via ZazaXpress are two separate paths. A lounge listing is no promise of sales or consumption on site; always check the house rules and the rules that apply.

FAQ: Frequently Asked Questions

Am I Guaranteed a Prescription?

No. A prescription is not guaranteed. It can only be issued after medical treatment and once all criteria are met, and the treating doctor alone decides. Express pickup or delivery* is only possible with a valid prescription. Availability, delivery times and pickup depend on location and pharmacy.

Can I Drive While Taking Medical Cannabis?

This is one of the most common and complicated questions. As a general rule, people taking prescribed cannabis may drive as long as they are not impaired behind the wheel. In other words, you must be able to drive safely. If the police stop you, you must be able to show that you are a medical cannabis patient, for example with a copy of your prescription or a patient ID card. Even so, a positive THC drug test can cause problems. Be sure to discuss this with your doctor, who is best placed to explain what you need to bear in mind.

Can I Travel Abroad with My Prescription?

Travelling with medical cannabis is possible, but it takes preparation. Within the Schengen Area, you need a certificate under Article 75 of the Convention Implementing the Schengen Agreement. Your doctor issues this certificate, and the relevant state health authority must certify it. There are no uniform rules for travel outside the Schengen Area. You must contact your destination country's embassy in advance to check the applicable laws. Never simply pack your cannabis and go!

What's the Difference Between Medical and Recreational Cannabis?

Medical cannabis dispensed by pharmacies is subject to extremely strict quality controls. It is grown under Good Manufacturing Practice (GMP) standards and is tested for pesticides and contaminants. Its THC and CBD content is standardised, allowing precise dosing. Recreational cannabis, such as that available through Cannabis Social Clubs or grown at home, is not subject to these pharmaceutical requirements. Medical cannabis is a medicine used to treat symptoms, while recreational cannabis is used for enjoyment.

How Do I Find the Right Strain and Dose?

That's your doctor's job. Treatment always follows the principle "start low, go slow". This means beginning with a very low dose and gradually increasing it until you achieve the desired effect without significant side effects. Your doctor will choose a strain with an appropriate THC-to-CBD ratio and specify the exact dose on your prescription. Finding the best approach for you is a gradual process that you and your doctor work through together.

In Short

Medical cannabis in Germany always starts with medical treatment. A prescription is not guaranteed; it can only be issued if the treating doctor decides so once all criteria are met. With a valid prescription, ZazaXpress offers express pickup or delivery* in several cities. Good preparation helps you in the consultation.

Although Germany's system is tightly regulated, it shows that cannabis is recognised as a legitimate medicine.

*Applies only with a valid prescription. A prescription is not guaranteed: it can only be issued after medical treatment and once all criteria are met. Processing time, availability, delivery times and pickup depend on location and pharmacy.
