Medical Cannabis Costs in Germany: Insurance & Self-Pay 2026

How Much Does Medical Cannabis Cost? Costs, Insurance and Self-Pay Options
Thinking about medical cannabis, or already have a prescription? Your next question is probably about cost. The answer depends on whether your health insurer covers the treatment or you pay for it yourself. Here is a look at the costs that can arise with medical cannabis in Germany.
*Important: A prescription is not guaranteed. It can only be issued after medical treatment and once all criteria are met. Whether that is the case is decided solely by the treating doctor. Fast pickup or delivery is only possible with a valid prescription.
Prescription Medical Cannabis: A Quick Overview
Since 2017 (and, since April 2024, under the Medical Cannabis Act), doctors in Germany have been able to prescribe cannabis medicines, including flower, extracts and finished pharmaceutical products, for serious conditions. The idea is to offer a treatment option when standard therapies do not work or cause intolerable side effects. Whether a prescription is issued is decided solely by your treating doctor after treatment and once all criteria are met. After that comes the question of how to pay for it. Broadly speaking, there are two options: health insurance coverage or paying out of pocket.
Getting Your Health Insurer to Cover the Cost
Ideally, your statutory health insurer (GKV) will cover the cost. If it does, you will only pay the standard prescription co-payment of €5 to €10. Getting approval, however, can involve plenty of obstacles and paperwork.
Before issuing your first prescription, your doctor must submit an application for coverage to your insurer. This application needs a strong medical justification. It must explain:
- What serious condition you have.
- Why recognised standard treatments are unavailable, ineffective or unsuitable for you.
- Why there is a reasonable prospect of a noticeable positive effect on the course of your condition or its serious symptoms.
The insurer will usually refer the application to the Medical Service (MD) for assessment. Some applications are rejected the first time around. An appeal, often supported by a more detailed medical statement, is possible. Whether that makes sense in your case is something to discuss with your doctor.
Self-Pay Prices: How Much Does Cannabis Cost at a Pharmacy?
If your insurer will not cover the cost, or you choose to pay privately, you bear the costs yourself. Pharmacy prices consist of several components.
Pharmacies charge the purchase price plus mark-ups. As a result, the price per gram of cannabis flower can vary by strain and pharmacy. Your pharmacy can tell you current prices.
Several factors affect the price:
- Producer: Companies such as Aurora, Tilray and Bedrocan have different pricing structures.
- Strain: Certain strains with distinctive terpene profiles or high THC content can cost more.
- Processing: Unprocessed flower is generally cheaper than extracts or more complex pharmaceutical products such as Sativex.
Monthly costs depend on the prescribed quantity and the strain you choose. Ask the pharmacy and your doctor for a cost estimate in advance.
Getting a Prescription: Telemedicine as a Modern Alternative
Finding a doctor who prescribes cannabis is not always easy. Many practices are reluctant to prescribe it or have little experience with it. Telemedicine platforms offer a modern and often more straightforward alternative. You can register online and first complete a questionnaire; a video call with a specialist doctor is voluntary at ZazaXpress.
These doctors are familiar with cannabinoid therapy. At ZazaXpress, your treatment request is usually processed within 15 to 60 minutes*. Whether a prescription is issued is decided solely by the doctor, after treatment and only once all criteria are met. With a valid prescription, the medicine is ready for pickup at the pharmacy* or can be delivered*. ZazaXpress offers express pickup or delivery* in Hamburg, Bremen, Berlin, Cologne, Osnabrück, Dortmund, Augsburg and Leipzig; in all other cities the medicine is shipped by DHL with a valid prescription, so it can be pre-ordered*. Processing time, availability and delivery time depend on location and pharmacy.

Cultivation Associations (Social Clubs) vs. Medical Cannabis
Since Germany's Cannabis Act (CanG) came into force in 2024, cultivation associations, often called Cannabis Social Clubs, have also become an option. It is important to understand how they differ from medical cannabis providers.
- Cultivation associations: These grow cannabis for recreational use and distribute it to adult members resident in Germany (one membership per person). Quantities are limited, for example to 50 g per month. Although quality standards apply, these are not the strict pharmaceutical Good Manufacturing Practice (GMP) guidelines.
- Medical cannabis: This is a medicine. It is grown under GMP conditions, tested for microbes and pesticides, and often irradiated. It is dispensed only by pharmacies on prescription, with the strain and dosage tailored precisely to your medical needs.
Tempting as it may sound, as a patient you cannot simply replace your prescribed treatment with cannabis from a cultivation association. The standardised quality and active ingredient profiles of medical strains matter for medically supervised treatment.
Cultivation associations are open only to residents of Germany and are not an option for visitors. Zazapass lists smoking lounges and 420 events in Germany; you can find the current locations on our site. Each venue independently decides admission under its own rules and applicable law and may refuse a visitor.
FAQ: Common Questions About Costs
Am I guaranteed a prescription?
No. A prescription is not guaranteed. It can only be issued after medical treatment and once all criteria are met, and the treating doctor alone decides. Express pickup or delivery* is only possible with a valid prescription. Availability, delivery times and pickup depend on location and pharmacy.
Does private health insurance (PKV) cover the cost?
That depends on your individual policy. Ask your insurer in advance whether and under what conditions medical cannabis is reimbursed.
Can I claim the cost for tax purposes?
That may be possible depending on your case. Spending on doctor-prescribed medicines can in some circumstances be treated as an “extraordinary expense”. Keep prescriptions and receipts and clarify the details with a tax adviser.
Why can the costs be high?
Several factors influence the cost. Cultivation requires highly sterile, controlled conditions to ensure pharmaceutical quality, which is complex and expensive. Add to that the costs of laboratory testing, irradiation to kill microbes, packaging, imports, wholesale distribution and, finally, legally regulated pharmacy mark-ups. Together, these stages influence the price at the pharmacy.
Are cheaper strains available at pharmacies?
Yes, prices vary by strain and producer. It is worth discussing this openly with both your doctor and your pharmacist. Sometimes a cheaper strain can offer comparable therapeutic benefits. Some pharmacies specialise in cannabis and can offer expert advice, or may stock particular strains at better prices. It never hurts to ask!
